Do you accept insurance?

Yes! We currently accept the following insurances:

BCBS: Medicaid, Medicare, & Commercial Plans

Presbyterian: Medicaid, Medicare, & Commercial Plans

Molina: Medicaid & Commercial Plans

NM Medicare or “Original Medicare”

Not currently in network plans/options:

United: Please note that currently United has denied our contract multiple times and we have no timeline on when/if we will have a contract. Please review our superbill options and reach out to our team with any questions. You can also file an official complaint to United regarding the delay in our contract.

Tricare: We do not have an active contract with Tricare but they are often flexible and willing to approve each visit as long as a prior authorization has been approved. Some plans allow the patient to request the prior authorization straight from TriWest, other plans require the Primary Care Manager (PCM) to submit the authorization. It is also variable depending on if you are the service member or a dependent. We recommend calling TriWest and asking what is needed for your specific situation. The customer service number for TriWest is 888-TRIWEST (874-9378). You can also often use our superbill option and get reimbursed by Tricare after your visit.

We are always willing to try to navigate insurance questions/concerns with you. It is often beneficial to call your insurance and inquire with them directly as well. Please see the below FAQ for some additional information/details that your insurance company may need.

How can I verify my insurance coverage?

We always recommend that you call your insurance to get an estimate of what you might owe. Your copay, coinsurance, or deductible may vary depending on many factors. Your insurance company may ask for some of the information below.

Although we do work as Menopause New Mexico all of our billing goes through our overarching company, Clinical Assessment Group. When speaking to your insurance company always utilize the name Clinical Assessment Group and clarify which provider you are seeing within our organization.

  • CPT Codes: 99205, 99204, 99215, 99214, 99213
  • Jane Epstein NPI: 1073528535
  • Clinical Assessment Group NPI: 1063862241

Your insurance company can give you the most accurate information about your specific plan.

What if you do not accept my insurance, or I am uninsured?

We also offer a self-pay option. You can still use your insurance for other services, such as labs or imaging, if you choose. Below are our current rates, which are subject to change. We have all patients sign a private-pay estimate before their appointment. If you need a payment plan, please contact our team.

  • Intake Appointment: 60 minutes: $250
  • Follow-up: 45 minutes: $150
  • Follow-up: 30 minutes: $115
  • Follow-up: 15 minutes: $75
  • Annual Visit: 60 minutes $200
  • Individual Counseling: $150
  • Couples Counseling: $175

You can also submit a superbill to your insurance for possible reimbursement.

What is a superbill and how does it work?

superbill is a document that summarizes your visit, including provider information and diagnosis codes. Some insurance plans allow patients to submit this document for out-of-network reimbursement.

After your visit, you can submit the superbill directly to your insurance company through their online portal or by mail. If your plan offers reimbursement, the payment is typically sent directly to you.

How do payments work?

We offer a convenient virtual invoicing option. You will receive a text and/or email with a link to pay your bill. You can also call us or pay your bill in person. We accept cash, cards, and checks in person.

How do I complete my intake forms?

You will receive a link to complete your intake forms shortly after booking your appointment. We ask that all patients complete these forms 3 business days in advance to allow our team to properly prepare for your visit.

New patients have 4-6 forms to complete that should take 10-30 minutes.

Follow-up patients have 1-4 forms to complete that should take 5-10 minutes. We have you complete these forms at every follow up appointment to help track your symptoms and progress.

We use Tricore and Quest for all lab work. You should not need a physical order form, but we are happy to send you one if you would prefer, just let us know!

How do I request medication refills?

You simply need to send Jane a message in the patient portal. Please include the medication information and verify the pharmacy you want it sent to. It is helpful to book a follow-up appointment if you need to make changes to your medication or have any concerns.

How should I communicate with the medical team?

The best way to communicate with our clinical team is through the Follow My Health patient portal. This allows us to securely review your message, access your medical information, and respond appropriately.

For administrative questions such as scheduling or billing, you may call the office directly at (505) 600 1735. Leave a message, and we will return your call.

How should I upload records to the patient portal for my provider to review?

Due to the fact that the Follow My Health patient portal can be shared with multiple clinics/providers simply uploading documents to your portal does not share it with any specific provider. We cannot view anything that you upload to the portal.

You must send Jane or Ashley a message in Follow My Health with the records/documents attached via the paperclip icon within the messaging feature. From there we can easily add these documents to your chart with our clinic.

If you need any assistance please reach out to our team. Below are instructions on how to send a message in the portal!
